The Golden Rule: Everything that touches your beer AFTER the boil must be sanitized. The boil kills bacteria; after that, it's your job to keep things clean.
Before Brew Day (Clean)
Brew pot (wash with hot water, soap OK)
Mash paddle/spoon
Grain bag (rinse, inspect for holes)
Thermometer
Any measuring equipment
Note: These items only need CLEANING (soap and water). They touch wort before the boil, so they don't need sanitizing.
During/After Boil (Sanitize)
Fermenter (jug or carboy)
Airlock
Stopper/bung
Funnel (if using)
Strainer (if using)
Any spoon used after boil
Hydrometer and test jar (if taking OG)
Bottling Day (Sanitize)
All bottles (12+)
Bottle caps (soak in sanitizer)
Bottling bucket or vessel
Bottling wand/tube
Siphon/auto-siphon
Tubing
Bottle capper
Small pot for priming solution
Measuring spoons
Sanitizer Quick Reference

Iodophor: 12.5 ml per 5 gallons. No rinse at correct dilution. 2 minute contact.
Bleach: 1 tbsp per gallon. MUST rinse with boiled/sterile water.
Pro Tips:
Keep a spray bottle of Star San handy during brewing
Don't fear the foam - it's safe and effective
When in doubt, sanitize it
Clean first, then sanitize - sanitizer doesn't work on dirty surfaces
